What kind of cover do you need?
Check all that apply.
Ex: IngramSpark, Kindle Direct Publishing, Draft2Digital, etc.
Note that this needs to be finalized before I can begin designing. If you're unsure of what size you want, I recommend measuring books from the library and then checking if your printer offers those dimensions (not all printers offer all sizes).
An exact spine measurement directly from your printer is always preferred, but some printers (like KDP and IngramSpark) calculate spine width based on final page count and paper weight. Note that the final page count is not the number of pages in your Word document, but of the final interior layout created by your typesetter. If you don't have a final interior layout, you can give me an estimate by taking your manuscript's word count and divide by 300.
What type of paper will your book be printed on?
"Creme" is usually standard for fiction. "Premium color" is standard for full-page images.
Ex: Endorsements or awards
This does not have to be final, but it helps me if I have something to work with.
Do you want an author photo included on the back cover?
Ex: Endorsements, publisher information, calls to action, etc.
The designer will credit her name on the back cover and would kindly request the author also place credit on the copyright page of the book.
This is not the place for your back cover synopsis (which you've already included above), but rather a place to explain the full contents of your book. The more I can understand the plot or purpose, the better I can make a design to match. If your book is a novel, feel free to give a basic overview of the story—spoilers included. If your book is nonfiction, explain some of the topics and/or chapter titles that you’ll include.
If desired, give a brief description of your major characters. This can include physical features and character traits that may help me if one or several characters appear in the design. (Pinterest board links are welcome for reference.)
If your book centers on an important symbol, image, item, or theme that could appear in the design, list them here. (Pinterest board links are welcome for reference.)
Do you have any ideas of your own you’d like to see tried in the design?
Provide links to specific covers of other books you like the design of. (Or provide a link to a Pinterest board). I recommend providing at least five covers.
Are there other books that are similar in style or feel to yours? Knowing the “vibe” of your book in comparison to others often helps me with the feel of your design.
List any other information you'd like me to know.

Note that this form is not a contract, nor does completing it guarantee a commitment from me. For various reasons, I can't take on every project that comes my way, but I'll do my best to meet your needs or refer you to someone who can.
